Registration Forms

New patients can save time at their initial visit by completing registration forms at home. To obtain forms, please call us (802-229-0033).

Appointment Reservations

Call our Montpelier dental office to schedule an appointment with Dr. Forest. We are open Monday through Thursday, 7:30am to 4:30pm and Fridays (twice a month), 8am to 3pm. 

Insurance and Billing Information

We accept most insurance plans.  Our insurance experts will work to help you receive the maximum benefit from your dental insurance carrier. We submit electronically to insurance companies to expedite claims for payment and, upon request, offer pre-authorizations for treatment. At the time of service, expect to pay your deductible and/or estimated portion of treatment not covered by your insurance. After your insurance carrier remits payment, you will be responsible for any remaining balance on your account. Please feel free to call us with questions at any time.
